00 an hour 7 days ago 7 days ago 7 days ago The Activity Coordinator (Community Engagement) is responsible for the daily supervision, implementation, and case management of all individuals participating in Shen-Paco's Group Day Support Program.
Hours Monday-Thursday 8:
30am-2:
30pm.
Essential Functions:
1.
Develops and implements individuals' activities designed to increase development capabilities and functional skills in accordance with Individual Service Plan (ISP).
2.
Ability to schedule outings and share schedules with appropriate staff.
3.
Maintains timely, accurate and complete documentation and files according to policy and licensure requirements.
4.
Must request and manage funding from group homes quarterly for individuals' community engagement spending.
5.
Communicates with individual/family/caregiver/case management about any needs pertinent to the individual.
6.
Secures medications and makes available to individuals per Shen-Paco's policies (If Medication Management trained) 7.
Works with Activity Staff to provide supervision and maintain safety of all individuals.
8.
Communicates regularly with Program Director, Facility Supervisor and Activity Supervisor about any important issues dealing with individuals.
9.
All other duties as assigned by management.
